Instructions:

How Do I Get Reimbursed For Medical Expenses?

  1. You will only need to request a reimbursement after your share of the deductible has been met.
  2. To get paid for incurred medical expenses, fill out a reimbursement form and send in with a copy of the explanation of benefits (EOB) from the insurance company. You can download the form below.
  3. Prescription drugs require payment at pickup; send reimbursement form and receipt ASAP to get money ASAP.
  4. Fax, e-mail or mail a copy of the EOB and reimbursement form to:

  5. Reimbursements are processed every Thursday and checks mailed on Monday.Best to get request in by Wednesday.
  6. The reimbursement should not be deposited into your health savings account (this is already tax free money to you.
  7. You should be reimbursed for medical expenses before you need to pay the providers bill.
