- Incur a qualified expense and pay provider with your own money.
- Fill out “Member Reimbursement Request Form”
- Mail, fax or e-mail receipt showing provider name, address and amount with Member Reimbursement Request Form.
- For monthly health club memberships, we have an automated payment program. Include a copy of your contract and fill in the banking information on the Member Reimbursement Request Form.
- We will process reimbursements every Thursday and mail checks by Monday.
