Products:

$0 Personal Deductible 0% cost Share -  Member has a high deductible health plan from an insurance company $5,000 single or $10,000 family.  Member has a $0 personal deductible for all medical expenses.  Member pays nothing of insurance company’s deductible.  Medical Claims Manager (Healthy By Choice) pays 100% of insurance deductible.  Member's annual maximum out-of-pocket for medical expenses is $0.  Some plans may have prescription drug copayment.

$0 Personal Deductible 25% cost Share -  Member has a high deductible health plan from an insurance company $5,000 single or $10,000 family.  Member has a $0 personal deductible for all medical expenses.  Member pays 25% of insurance company’s deductible ($5,000 single / $10,000 family) as they incur medical expenses.  Medical Claims Manager (Healthy By Choice) pays other 75% of insurance deductible.  Member's annual maximum out-of-pocket for medical expenses around $1,250 for a single or $2,500 for a family.

$250 Copay Plan – Member has a high deductible health plan from an insurance company $5,000 single or $10,000 family.  Member pays the first $250 of each medical expense incurred.  Medical claims manager (Healthy By Choice) will reimburse member only if a medical expense cost more than $250 for each item.  Member's annual maximum out-of-pocket for medical expenses could be $250 or up to $5000 single or $10,000 family.  Member pays the first $250 for each and every medical expense until total medical expenses reach the insurance deductible of $5,000 single or $10,000 family.
